Community care

Wherever possible Pilgrims Hospices is able to offer specialist palliative care to patients outside the services offered by its three dedicated hospices.

By advising patients in their own homes, liaising with other health and social care colleagues and agencies we make it possible for many of our patients to live alongside family and friends whilst receiving the vital care they need from our team of specialist carers.

We also have a key role in helping our professional colleagues working in the community eg GP's and District Nurses offering them our best specialist advice and education so that they can also help patients to best effect.
